Survey ELIGIBILITY REQUIREMENTS
PARTICIPANTS MUST BE
One of the Randomly Selected Households in the 48601, 48602, or 48607 ZIP Codes of Saginaw; AND …
Have a member of the household that is 12 years old or younger; OR …
A Household with a pregnant household member; OR …
Households where the person answering the questions for the household is at least 18 years old, and either the parent/guardian of at least 1 child under 12 in the home or a pregnant woman.
WHY SHOULD I PARTICIPATE?
For completing the survey your household may qualify for one of 15 gift cards of $100 each.
Households who will receive these gift cards will be selected randomly from the 1500 households that are participating in the study.
In addition, your household may qualify to participate in the next phase, the intervention phase, of the program where each household selected will receive products (i.e., a HEPA vacuum cleaner, fans, smoke detector, carbon monoxide detector, and other items) worth about $450 or work done on your home valuing up to $2,500 to reduce hazards in your home.
